We are a passionate team that supports people in their homes. We aim to maximising our clients’ independence, privacy, and dignity by supporting them to develop their skills. We value client empowerment and safety. Our team comprises qualified and experienced professionals which include Registered General Nurses, Social Worker, Project Manager, and Trainee solicitors.
The Experience Social Worker does the assessments in line with the Care Act. They work with the clients and their loved ones, to come up with a robust support plan that is tailor to each individual. The individual care plans are strictly adhered to, to ensure clients' needs are safely and adequately met.
The Social Worker also carries out Mental Capacity assessments where necessary.
