Home Care (Treatment of Disease, Disorder & Injury TDDI).

This is a is a specialist home care service designed to support individuals with complex medical conditions. We are CQC registered to deliver this complex service, and this support is Nurse-Led. Our Nurses have a wealthy of knowledge, skills and experience as they have worked in medical, surgical wards, hospice, dementia care, end of life care and learning disabilities. It is in these areas that they have attained experience and competences required for TDDI Lead Nurse care – and this is what makes us stand out as a care company.

Our TDDI Services include:

Palliative Care

This is the specialist kind of care that improves the patient’s quality of life and their families. It aims at preventing and relieves suffering from the terminal condition. Ad death and dying are not inevitable, Passionate Support Ltd support patients at home to live as comfortable as possible before death. We value and consider patients’ wishes and preferences.

Alzheimer’s and Dementia Care:

At Passionate Support Ltd, we design the support around the patient and your family as we are aware of the challenges dementia throws at families. We therefore provide dedicated Nurse-Led Dementia care in the comfort of patients’ homes. In the process, we promote independence, respect, privacy and dignity.

Our support is:

- Responsive: care is tailored to the patient's needs, and this is the focal point of supporting people with medical conditions. We ensure patient safety is paramount.

- Nurse-Led and has experienced carers.

- Emotionally supportive - as we aware how devastating the condition is, both to the patient and their loved ones.

- Catheter Care

- Stoma Care

- PEG Feeds

- Oxygen Therapy

- Pressure Area Care

- Support with Compression Stockings

- Administration of ear drops

- Administration of eye drops

- Administration of nebulisers and inhalers.

- Professional patient observations that are timed.

- treatment of a physical or mental health condition.

Giving vaccinations

Home Care (non- Complex Care).

This is the opposite if care home and/or residential care, as it allows you to be supported in your environment.

Our Services are as follows:

We are mindful that home care is not a one-size fits all service. We therefore provide person-centred care and support people in their homes with:

- personal care.

- cleaning teeth and brushing teeth., managing continence wear.

- Getting appropriately dressed

- medication.

- managing food and fluid intake. This also covers supporting people with hot balanced meals.

-promoting their independence and improving on their skills.

- ensuring they are in a safe environment.

- ensuring they are in a clean environment.

- laundry.

- budgeting and shopping.

- accessing medical appointments.

- accessing the community to be involved in the activities of interest, such as morning coffee, meals out, bowling, swimming.

- visiting friends and relatives.

- running some errands such as medication collection, collecting your groceries.
