What is this service?

 This is when we safely support young adults with with complex conditions in the comfort of their homes.

Our trained Nurses and carers work with adults discharged from hospital to:

  • Safely recover in the comfort of their home.
  • Support Improve their mental health and wellbeing as they will be nursed in the comfort of their homes and familiar environment.
  • Promote person-centred and continuous medical attention rendered by trained personnel.
  • Promote undivided support from staff.
  • Dedicate time to support to the patient with no rush. This is quality 1:1 support.
  • promote speedy recovery as the patient will be in a familiar environment.
  • Improve life quality and peace of mind for both the patient and family.
  • Promote patient and family choice and control so the patient can be supported at the times of their choice.
  • Cultivate the support with less stress levels in comparison to a hospital setting where many people are looked after by few staff.
  • reduced risk of infections as care is rendered by the same staff and in a smaller environment where infection control is effective.
  • support and reassure family members through explaining the patient’s condition, the treatment(s) they are on and the importance of liaising with the members of the multi-disciplinary team (MDT).

- PEG Feeds

- Support with Compression Stockings

- Administration of ear drops

- treatment of a physical or mental health condition.

Giving vaccinations

  • Additionally, we support the patients to:

- promote their independence.

- dignity.

- support them form and maintain relationships.

- improve their social skills, as well as improving skills related to activities of daily living. 

We also provide person-centred care in:

- personal care.

- cleaning teeth and brushing teeth.

- Getting appropriately dressed

- medication.

- managing food and fluid intake. We support people plan healthy meals, do food shopping, store food items appropriately and we actually do the cooking with them. 

- This helps them learn/improve the living skills and prepares them to be independent.

- ensuring they are in a safe environment.

- ensuring they are in a clean environment.

- laundry.

- budgeting and shopping.

- accessing medical appointments.

- accessing the community where they are supported to be involved in activities of interest. These include, swimming, bowling, gardening, having meals out, watching movies, visiting friends and family, going on holidays.
